Chronic Care Management
Chronic Care Management Program at Ashley County Medical Center
Chronic Care Management (CCM) is a program that is offered by Medicare for patients who have two or more chronic conditions such as arthritis, diabetes, depression, chronic obstructive pulmonary disease, heart disease, stroke, cancer or high blood pressure. This program was designed by Medicare to help us manage and coordinate your care outside of your office visits. Services include education, medication monitoring and interaction with you and your care givers concerning your health care needs.
